Form Tools  
Form Tools is written for web developers who work on online registration sites, or sites that require any form of information gathering from their online visitors. Put simply, it is a form processor, storage and data access script written in PHP and MySQL, designed to work with any existing web form. With a few minor changes to your form, you can stop using old-fashioned form-mail scripts and instead store form submissions in a database, instantly providing your clients with options such as mass data export via Excel, printer-friendly pages, data searching and sorting, form submission editing and optional email of form submissions to administrators and users. You can even add filters to each client account to limit what submissions they see based on any criteria from your form. Form Tools is available for free under the GNU public license. This is a complete webmail contact system, including a template contact form with javascript validation, and a php script to send the messages to an email address of your choice. Form2CSVTable
This application will convert all data submited by an HTML form to a CSV table located in the same folder as the script. The generated CSV file follows the RFC4180 standarts and can be opened in Excel or in any other Spreadsheet software.

Form2Excel and Mail
Form2Excel allows sending every field of a form filled by a user, into an Excel file formatted as CSV and all the form data is sent also into an email... even the uploaded files!. This product is available as Dreamweaver extension form MX and later versions, so you can insert and use it without requiring hand coding or advanced skills. This way you can check and manage easily the data from your web site forms using Excel. The uploaded files will be saved into a folder on your web site and Form2Excel will include a reference from the CSV file to the uploaded files. The uploaded files will be also included as attachments into the email. No advanced configuration required, the fields from the form are identified automatically. Additional info is identified automatically and saved into the Excel file: country, IP, browser, useragent, server time and referrer.

Form2Mail
Form2Mail allows to email the info of any HTML form, including uploaded files that are sent as attachment into the email, along additional technical and geographical information about the user sending the form identified automatically. The additional information covers the user's browser, IP address, country, the page where the form is located (if sent as referrer), server time and detailed user-agent info sent by the browser. FormBuilder
FormBuilder is a PHP class that simplifies the process of creation and validation of HTML forms. It can also output the results in a text form and email them. FormBuilder in essence manages a collection of field classes that implement a standard interface defined by the FormField class. Classes currently implemented include text fields, radio buttons, checkboxes, popup menus, date/time, etc. The generated form can be fully customized through CSS. The current version of FormBuilder is already very usable, but there are some improvements in planning: Better validation, more types of Fields and database integration.
